Dear Parents and Carers,
Please complete this form if you would like to attend the Parent & Staff quiz night on Thursday 20th June, 6:45-8:30pm in the theatre at Chelsea Academy.
If you are coming as a team please add the names of other parents/staff in your team. If you are coming alone you can join another table.
The event will not be catered, instead it is a “bring and share” event where adults in the team bring refreshments to share on their table.
No children should attend this event.
